APCC - Australian Primary Care Collaboratives

The Australian Primary Care Collaboratives affectionately known as APCC to most is a program that support practices in continuous quality improvement in primary care. After much deliberation, 3 practices were chosen from varying backgrounds to be part of this journey through the Division.

The Model for Improvement utilises the Plan-Do-Study-Act concept where three key questions are asked before embarking on a change, they are:
1. What are we trying to accomplish?
2. How will we know that a change is an improvement?
3. What changes can we make that will result in improvement?

2 representatives (usually 1 clinical and 1 practice support) from each practice are skilled with the required knowledge through a series of workshops over the 18 month period where time between workshops is allocated for the representatives to introduce The Model for Improvement concept and changes to their individual practices.
