Chronic Disease

 The Chronic Disease Program focuses on activities that maximise the impact of general practice on the prevention and treatment of chronic diseases. More specifically, the program focuses on supporting and promoting co-ordinated team based activities involving all members of general practice.

Activities focus on Diabetes, Enhanced Primary Care and Self Management. All of the above activities and projects within this Program are guided by research and investigation of Evidence-Based Medicine, liaison with general practice, government/non-government health agencies and community organisations. This ensures that all the activities are appropriate, and closely reflect and address local and current needs of Capricornia GPs.

The Division is setting up a Diabetes Type 2 patient focus group. This group will meet on a monthly to bimonthly basis where patients can receive some general diabetes and lifestyle related education and also network with other patients who have diabetes type 2.

Enhanced Primary Care Initiatives of the Federal Government are also a priority of the Division. The Division aims to assist GPs and practices to complete these items to improve the coordination of patient management and ultimately improve patient care and outcomes of patients with chronic disease.
