The Integration Program looks at ways of improving communication and service delivery across the health care sector by:
- Providing linkages between Practices, Hospitals, Government and the Community to disseminate information, facilitate programs and address identified issues.
- Participating in Community Service Provider forums and strategic planning for local organisations including Queensland Health. This provides the opportunity for the Division to input into the strategic direction of the local Primary Health Care System.
- Facilitating regular meetings to address specific local issues to improve the community response and client care across the continuum, improving health outcomes and the use of resources. Examples of these are the Rockhampton Sexual Assault Response Group, the Aged Care Panel and the Chronic Disease Management Group (SWICC Staying Well in Capricornia).
- Taking the GP perspective to the Hospitals and other Health organisations to ensure that GPs are included in the coordination and care of their patients.
- Assisting to address identified issues. This could include improving clinical pathways, facilitating meetings between key stakeholders, or developing new programs or activities to meet identified needs.
